In Reply First, as stated in the JAMA Guide to Statistics and Methods article, case-control studies provide the estimate of the OR rather than the RR because the occurrence of the outcome in the population (incidence or prevalence) usually cannot be estimated. The RR can be obtained in cases in which the occurrence of the outcome in the population can be estimated, as the authors of the letter point out.
